Instructions for Completing your Application

Applicant Information

The information you provide will be used for the membership directory, your membership certificate, all correspondence and promotional material, and for addressing the PAAO journal Vision Pan-America.

Certification

Please print the names of the country(s) in which you are licensed to practice. A copy of your membership certificate or a letter from the affiliated society must accompany your application for Active membership.

Application Endorsement

An endorsement form is located on the back of the application form. This endorsement must be completed by a PAAO Active Member (Miembro Titular), or Life Member.

Membership Dues

(all prices in US$)
Active Member dues of $150 must be submitted with your application. This covers your membership dues from January 1 through December 31. Dues are based on a calendar year. Payment must be made in United States currency. Accepted forms of payment include cash, check, International MasterCard or Visa, American Express, and wire transfers. Additional membership categories include Member-in-Training (residents and fellows with letter) $50 and Corresponding (members outside the Western Hemisphere) $100, Affiliate Member $75 (A person who holds a degree of Doctor of Medicine, Doctor of Osteopathy, or Doctor of Veterinary Medicine, or Doctor of Philosophy, and who is not an ophthalmologist but is engaged in a field allied with or in a basic science related to ophthalmology).
